| A: Yes, new patients are always welcome and scheduled without delay. |

| A: We accept Aetna PPO, Blue Cross Blue Shield PPO, Cigna PPO, United Healthcare PPO, Private Health Care Systems (PHCS) and Medicare. |

| Q: I do not have insurance, how much is it for an initial office visit if I have to pay cash? |
| A: An office visit can cost anywhere from $200.00 to
$725.00 depending on extent of injury and treatment required. |

| Q: What forms of payment do you accept? |
| A: We accept cash, checks and all major credit cards. |

| Q: Do you accept Workers' Compensation Insurance? |
| A:
Yes, Dr. Berliner and his staff are very experienced with the Workers' Compensation rules/laws. |

| A: Monday’s and Thursday’s at North Freeway from 9:30 AM to 3:30 PM and Wednesday’s and Friday’s at Gulf Freeway from 9:30 AM to 3:30 PM |

| A: Yes, Dr. Berliner treats minors 2 to 18 years of age. |

| A: Yes, just bring the forms to your appointment that is required to be filled out by your athletic department. |
